Residential and Community-Based Services

Indian Creek Foundation’s Residential and Community-Based Services department provides a continuum of individualized services to support adults with intellectual/developmental disabilities or autism in exploring their place in the community. With multiple service models available, this program provides person-centered planning to meet the needs of the individuals served.

Residential Services

Care is provided in group homes or individual apartments throughout the area. All residents have choices to participate in community activities such as shopping, banking, dining, attending the church of their choice, etc. Direct Support Professionals, working on a shift basis, provide support tailored to the specific needs and preferences of the individuals we serve. Service models consist of a licensed setting that provides 24-hour-a-day support and unlicensed settings that provide less than 30 hours per week of support. Both models include medical oversight by our Nursing Department.

Residential

Licensed Residential

Individuals requiring a high degree of structure, support, and care to assist them with mastering the basic activities of daily living are served in a Licensed Residential care setting. Some individuals in this setting are striving for semi-independent living. They will learn such tasks as general housekeeping, meal planning and preparation, handling money, and support to navigate their community.

Unlicensed Residential

Individuals capable of living in a semi-independent living situation focused on increased responsibility and self-reliance are served in an Unlicensed Residential care setting. Some individuals in this setting have progressed from Licensed care to independent living and will receive ongoing support and monitoring.

Lifesharing - Licensed and Unlicensed

This service model offers Lifesharing through community family living. Lifesharing offers care for a person with intellectual/developmental disabilities or autism by a host family in their private home in the community. Lifesharing provides a stable, supportive environment in which each person is encouraged to reach his or her fullest potential while becoming part of the family’s everyday life. The Lifesharing provider also provides food, shelter, and care for the person. Indian Creek Foundation is accepting Lifesharing Providers who would like to share their homes and provide care.

Respite - Adult Licensed Residential or Private Setting

Respite care provides families with short-term relief to have an opportunity to take vacations, manage personal matters, or just simply have a rest from their caregiving responsibilities. This program is offered to adults with intellectual/developmental disabilities or autism and is provided in a Licensed Residential group home or a private setting. Indian Creek Foundation is accepting Respite Providers who would like to share their homes on a short-term basis.

Nursing Services

Indian Creek Foundation nursing staff take an active stance in the areas of advocacy, protecting individual rights, respecting human dignity, promoting independence, and maintaining an optimal level of functioning. Nurses oversee the provision of healthcare for all individuals receiving Residential and Community-Based Services.

Nursing Clinic

The Nursing Clinic facilitates access to professional nursing services across the agency.  Services provided include treatment of minor illnesses and injuries, well-checks, quarterly assessment of clients, wound care management, skincare assessment, follow-up respiratory care, immunizations, monthly medical injections, and assessment of emergent symptoms as necessary.

 

Health & Wellness Initiative – The Nursing Clinic offers Health & Wellness where healthy lifestyles are promoted.  Clients are assisted in making healthy choices and adopting behaviors that can lead to improved health.  The services provided by our nurses and other clinicians include health screenings, maintenance of existing conditions, and health promotion activities that cater to the unique needs of our individuals.  Additional services include daily insulin administration, weight management, in-house dental and podiatry, and quarterly assessments of our senior citizen clients.  Nursing also coordinates training for our Direct Support Professionals to support the medical needs of the individuals served.
